An example of one possible way it could work might be that the sender thinks a conscious and intentional thought which is mentally labelled for receipt specifically by the intended recipient, and the recipient experiences this thought as a cognitive [[intrusion]] heard in the grammatical second person---but this is not the only possible approach. Different systems have different approaches, and some have none at all.
